This is a new program that aims to maximise the ability of socially isolated older or frail aged Bankstown residents over 65 (including those from CALD backgrounds), to continue to live independently at home.

Eligible people are those at risk due to factors such as low income, living alone, lack of access to transport, poor mobility and poor health status. They must also not be a recipient of a Community Aged Care Package (CACP) or an Extended Aged Care in the Home (EACH) Package

Initially we support an individual to complete a health assessment, which is used to identify his/her social, physical and emotional needs. This information is then used to identify and organise suitable support activities for the individual to participate in.

Activities include:

Linking an individual or a small group (3-4) with one of the established activities run at the Chester Hill Neighbourhood Centre or by other providers in Bankstown.

  1. Outings and day trips
  2. Friendly home visits
  3. Accompanying individuals to appointments and social outing
  4. A simple chat and a cuppa.

A member of the Social Support Team or an organised volunteer assists the individual with their chosen activities.

After 6- 9 months, we follow up an individual’s progress and also reassess their needs to determine if changes need to be made to their plan.

We also welcome volunteers who would like to help to provide these activities.
